    What are the hours for The Advisement Center?

    The Advisement Center is open Monday-Friday from 8:00 am to 5:00 pm; however, the center does close for devotional.

    Do I need an appointment in order to meet with an Academic Advisor?

    Our office is a walk-in center so no appointments are necessary. The advisors work with students on a first come, first serve basis. If you are coming from out of town or need to schedule a time to meet with someone, an advisor would be happy to make those arrangements.

    How do I change my major?

    This depends on your desired major. For:

    Exercise Science,
    Exercise and Wellness, or
    Athletic Training:
    Students will need to fill out a change of major form and meet with Farris or Gayle to go over the program Requirements.
    PE Teaching/Coaching:
    Students are put into the pre-program with a change of major form until they are accepted into the major. Once they are accepted into the major their major will change to PE Teaching/ Coaching. It is strongly recommended that students interested in this major meet with their faculty advisor and also with Farris or Gayle.
    Public Health:
    Students will need to get a Change of Major form, a “How to Change your Major” sheet, and a Public Health brochure from the Advisement Center. Once students have read over this information they will need to make an appointment with their faculty advisor, Stephanie Fugal in 221 RB.
    School Health Education:
    Students will fill out the Change of Major form in the advisement center and visit with a faculty advisor, either Randy Page or Cougar Hall. They must take: 1) Change of Major form, 2) Program Requirement Form, 3) ABC Report. Once the forms are signed they may then return them to the Advisement Center. A Personal Identification sheet must also be filled out and taken to 221 RB.
    Leisure Services Management:
    Students must first speak with their faculty advisor and have them sign a pink contract paper. After this form is signed, students may bring it to the Advisement Center and fill out a change of major form.
    Therapeutic Recreation:
    Students must complete a yellow packet from the department office (273 RB) and meet with their faculty advisor. They must also have the pink contract paper signed, and then they may declare their major with a Change of Major form in the Advisement Center.
    Dance:
    All dance majors are put into the pre-program with a Change of Major form. Once they have been accepted into the major, the advisement center will receive a list of accepted students. Please check with the Dance Department (294 RB) for more information.

    How do I declare a minor?

    In order to declare a minor in our college, students must obtain a minor clearance form from the advisement center. They must obtain a signature from the minor advisor (whose name is located on the bottom of the clearance form) and then return the form to their own major advisement center (if the student is part of our College( HHP), return the form to us).

    When do I apply for Graduation?

    It is suggested that students meet with an advisor no less than three semesters before they plan on graduating to discuss their status for graduation and to make sure they are on track to graduate when they plan on graduating. Application deadlines are:

    April Graduation
    November 15 (The November prior to the applicable April graduation)
    August Graduation
    April 15 (The April prior to the applicable August graduation)
    December Graduation
    Septemeber 15 (The September prior to the applicable December graduation)

    *It is recommended that students apply for graduation as soon as they have registered for their final semester rather than wait for the deadline.

    How do I apply for Graduation?

    In order to apply for graduation students will need to come into the advisement center. An advisor will then meet with the student and go through his/her progress report and graduation application. Once this is complete students will need to turn in their graduation application, along with a $15.00 fee, to D-155 ASB.

    How do I register for more than 18 credits during Fall/Winter or 9 credits during Spring/Summer?

    In order to register for more than the allotted amount, students will need to obtain and fill out an add/drop card (these are available in the advisement center or at One Stop). Students will need to get a signature from the instructor of the course(s) that will put them over hours. Once this form is complete, students must come to the advisement center and meet with an academic advisor. The academic advisor can approve up to 21 credits for Fall or Winter semesters, or up to 11 credits for Spring or Summer terms (If a student is requesting more than the above amount of credits they will need to go to the petitions office). Finally, these forms are turned in to One Stop or the Registration Office.

    Where do I find my Major Academic Plan (MAP) online?

    1. Go to the BYU homepage (byu.edu)
    2. Select the tab labeled “Students”
    3. Select the tab labeled “Academic Links”
    4. Click “Advising”
    5. Under “Major Academic Plans” select the year you entered the program
    6. Select the college (Our college is The College of Health and Human Performance)
    7. Select the major

    Where do I find my Progress Report On-line?

    1. Go to MYMAP
    2. Click on the Plan Tab
    3. Select Progress Report (on the far right side next to the Explore other Majors/Minors tab)
